I hereby authorize this company to investigate, through whatever means deemed appropriate by the company, any information included to this company and facts resulting from the investigation unless otherwise noted. This company is also authorized to use any information obtained from its investigations to determine my suitability as a business. I release this company from any liability in connection with the investigation. 

I hereby authorize each this company to open my business file to a representative of this company.
 

I hereby authorize any former businesses, or any other persons given as references (unless otherwise noted) to answer any questions that may be asked.

If services are provided, I agree to abide by the policies, procedures, rules and regulations of this company. Furthermore, if any services are provided, I understand and consent to the fact that any personal and business information collected by the company during the services provided and in the course of my use of these services will be used as required in the management of the business relationship between myself and Knight's Armour. This includes the use of my personal email address for the purpose of furnishing me with my pay statement and for communicating with me for any other purpose associated with my employment. This information will be safeguarded and only used or disclosed in compliance with the Personal Information Protection and Electronic Documents Act and with CP's Privacy of Information Policy #1804.
 

Unless otherwise specified in writing, I understand that nothing contained in these services provided or in the granting of use of services and terms is intended to create a contract between this company and myself or business operated by a member for either contract services or for the providing of any benefit.

I understand that submission of this information exchanged does not obligate this company in any way.
By electing to use Knight's Armour services, I also consent to sign, complete and receive electronic documents relating to my business's profile, documentation, and going concern during both this session and any future sessions relating to my business's profile and documentation. Additionally, I consent to electronically receive communications relating to my business's going concern, including requests for additional information.

My consent applies to documents completed, signed or provided via this website, as well as to documents transmitted via email.

I understand that I have the right to withdraw my consent at any time by calling Knight's Armour (816) 558-5505. After withdrawing my consent, a company representative will contact me to coordinate arrangements to receive paper copies of documents and communications.

After consenting, I understand that I can obtain copies of documents and communications relating to my application by using my browser to print paper copies or save electronic copies of documents or screens during this session and future sessions; or by calling the Knight's Armour (816) 558-5505 to request that paper copies be mailed to me at no charge.
